Elevate Energy designs and implements efficiency programs that lower costs, protect the environment, and ensure that the benefits of energy efficiency reach those who need them most.

Medicare Medicaid Alignment Initiative (MMAI) Enrollments

The Medicare Medicaid Alignment Initiative (MMAI) is a new program for older adults and people with disabilities who have Medicare and Medicaid. Instead of receiving your healthcare services through Medicare and Medicaid, you can receive all of your services through a health plan of your choice.  An MMAI health plan will cover all of your medical care, including doctor visits, hospital care, prescription drugs, and mental health services. If you get care in your home through the Community Care Program or the Department of Rehabilitative Services (DRS), these services will be covered by your    MMAI plan, too. If you get care in a nursing home that is paid for by Medicaid, that will also be covered by an MMAI plan. Meet with an Advocate from Solutions for Care that will help you chose and enroll in an MMAI provider or opt out of this program.

Journey of Grief – Support Group

Coping with the death of a loved one can be difficult and painful.  Spend time with other individuals who have experienced a similar loss.  This will be a time of sharing, encouraging, perhaps even crying, but all with the purpose of helping you through this period of life.  You are not alone in your journey of grief.  The session will be led by the professional staff of Season’s Hospice and Palliative Care.

Respect.  All of these things are important to you, but how do you tell your family about the place you want them to play in your care?  Join Staff from Season’s Hospice and Palliative Care to learn about the benefits of advance care planning by creating Advance Directives. Illinois State Law will be examined and acceptable advance care planning options will be discussed at length. Case examples will be provided.
