Senior Farmers Market Vouchers

The Senior Farmers’ Market Nutrition Program (SFMNP) provides qualified Cook County senior residents with vouchers for fresh fruits and veggies, herbs, and honey to be used at farmers’ markets throughout suburban Cook County. You must live in suburban Cook County, be 60 years of age and older and meet low-income qualifications. Booklets are limited and available on a first come, first serve basis. Professional Staff from Solutions for Care can assist you in determining your eligibility and completing the SFMNP application.

Journey of Grief – Support Group

Coping with the death of a loved one can be difficult and painful. Spend time with other individuals who have experienced a similar loss. This will be a time of sharing, encouraging, perhaps even crying, but all with the purpose of helping you through this period of life. You are not alone in your journey of grief. The session will be led by the professional staff of Season’s Hospice and Palliative Care.

Understanding Dementia

This presentation will review dementia, its causes, and interventions. Treatment decisions and options regarding aggressive care benefits and burdens are explained. Caregiver challenges and both pharmacological and non-pharmacological treatments for behavior problems in later stages are presented by professional staff from Season’s Hospice and Palliative Care. Learners will have an enhanced appreciation for the challenges of care, interventions available and the sensitivity required to care for patients with Dementia. Also, discussion on advanced care planning and the Five Wishes program and the timeliness of making arrangements in the early stages.

Back Flexibility Screenings

Back Flexibility Screenings utilize a “sit-and-reach” motion to measure the flexibility of the lower back and hamstring muscles.  The screening takes about five minutes.  Participants need to wear loose, comfortable clothing.
